Anticoagulation therapy management by the pharmacist has become the standard of care for patients in acute and ambulatory settings. It is imperative to ensure the availability of anticoagulation educational programs focused on contemporary, evidence based knowledge and skill development for pharmacists.

Description

This fourth in a series of 4 webinars will provide the participant with a foundational knowledge in the management of patients on anticoagulant therapy. The webinar will be followed by a case study that will allow the participant to apply knowledge gained during the webinar.

Target Audience

This program is intended for for pharmacists practicing in community or health system pharmacy who may be involved in the pharmaceutical care and monitoring of patients at risk for stroke from atrial fibrillation

Requirements

The program offers 6 hours (0.6CEU) of continuing pharmacy education credit. To receive credit, registrants must view the entire online webinar and complete the online activity evaluation. Statements of credit may be printed online at that time. No partial credit will be given.
