Anticoagulation therapy management by the pharmacist has become the standard of care for patients in acute and ambulatory settings. It is imperative to ensure the availability of anticoagulation educational programs focused on contemporary, evidence based knowledge and skill development for pharmacists.

The need for an anticoagulation practice-based activity for pharmacists is based on participant requests after attending previous educational programs and on the review of current literature and medical advances including, a review of changes in the treatment of Stroke Prevention in Atrial Fibrillation, the FDA committee approval of the first new class of drug to treat venous thromboembolism (VTE) in over three decades, and the National Patient Safety Goals.

Description

This third in a series of 3 webinars will provide the participant with a insight into the controversies surrounding anticoagulant therapy and a review of new agents.

This unique online case study is available to participants after each webinar. It will provide an opportunity for practitioners to apply the content learned during the webinar to simulated cases. Case studies will include online discussions with faculty and peers and are pertinent to community pharmacy practice as well as health-system pharmacy.

This activity is co-sponsored by ClotCare, a 501(c)3 organization. Go to www.clotcare.org for more information.

Target Audience

This program is intended for pharmacists practicing in community or health system pharmacy who may be involved in the pharmaceutical care and monitoring of patients at risk for stroke from atrial fibrillation and other conditions requiring anticoagulation.
