ONLINE - The Coming Revolution of Anticoagulation: A Practice-Based Certificate Program
Modules I, II, III, IV, and V
Anticoagulation therapy management by the pharmacist has become the standard of care for patients in acute and ambulatory settings. It is imperative to ensure the availability of anticoagulation educational programs focused on contemporary, evidence based knowledge and skill development for pharmacists. The need for an anticoagulation practice-based activity for pharmacists is based on participant requests after attending previous educational programs and on the review of current literature and medical advances including a review of changes in the treatment of Stroke Prevention in Atrial Fibrillation, the FDA committee approval of the first new class of drug to treat venous thromboembolism (VTE) in over three decades, and the National Patient Safety Goals.
Description
This continuing pharmacy education opportunity is unique because it is intended to provide a longitudinal approach to anticoagulation education for the management of Stroke Prevention in Atrial Fibrillation. Participants will be able to obtain knowledge-based continuing education from three on demand Webinars, apply their skills through online case studies, review and discuss controversies with anticoagulation therapy in a six hour on demand symposium, and enhance your practice using a practice-based model for developing unique care plans. When all modules are completed successfully in total, the participant will earn a practice-based certificate.
Target Audience
This program is intended for pharmacists practicing in community or health system pharmacy who may be involved in the pharmaceutical care and monitoring of patients at risk for stroke from atrial fibrillation and other thromboembolic conditions.
Schedule
Module I, II, and III - register, view, participate in active learning, then successfully complete the online requirements and post-assessment.
Module IV - Register, view online presentations, participate in the active learning and complete the online assessment.
Module V - Complete Modules I - IV as prerequisites for this module. Read all required documents and articles, development and implement a site specific anticoagulation therapy management plan, the complete the online assessment of learning.
Requirements
Complete first three modules (webinar and case study), view the presentations for Ansel Symposium, then complete the online self assessment and submit a site specific project. Up to 18 hours of continuing pharmacy education will be awarded for completion of this content. If all 5 modules are completed, a Certificate of Achievement will be awarded in addition to the continuing education credit. Partial credit may be awarded. Participants may print statements of credit for each module.
